Role & Responsibilities
The Assistant Nurse Clinician provides advanced and person‑centered nursing care to residents with complex needs in the nursing home. The role supports clinical leadership through supervision of junior nurses, coordination of multidisciplinary care, and participation in quality improvement initiatives to ensure safe, effective, and compliant care delivery.
Primary Responsibilities and Duties (80%)
. Resident Care and Clinical Duties: Conduct comprehensive nursing assessments and ongoing clinical monitoring for residents with complex and chronic conditions. Develop, implement, and review individualized nursing care plans. Assist with resident admission, transfer, and discharge processes. Prepare residents for procedures and ensure timely execution of physicians orders. Detect early signs of deterioration and escalate care appropriately.
. Coordination of Care: Provide guidance in coordination of care among interdisciplinary care teams and across care settings for residents with complex care needs.
. Clinical Leadership: Provide clinical supervision and mentorship to Staff Nurses and Enrolled Nurses. Support competency development, skills validation, and clinical coaching. Act as a clinical resource person within the nursing home.
. Quality Improvement: Support compliance with MOH regulations, and organizational policies. Participates in quality audits, incidents reviews and quality improvement project,contributes to the development and review of the nursing policies, SOPs and evidence-based practices.
. Training & Development: Percept junior nurses, newly hired staff and students. Precept and mentor junior nurses, newly hired staff, and contribute to in‑house training, clinical teaching, and community nursing education initiatives.
. Person-Centered Care: Promote and implement person-centered care that aligned with residents preferences, dignity, and psychosocial needs
. Resident Engagement: Engage residents and families to foster trust, shared decision‑making, and service excellence.
Secondary Responsibilities and duties (20%)
. Stay updated on current nursing, gerontology, and long‑term care practices
. Promote evidence‑based nursing interventions and continuous improvement
. Perform additional duties as assigned by the Reporting Officer in line with role scope
. Works closely with Senior Nurse Clinician and interdisciplinary team
Qualifications, Competencies & Experience
. Bachelor's degree in nursing. Specialist Diploma or Advanced Diploma in Gerontology is preferred
. Registered Nurse with valid Singapore Nursing Board (SNB) registration
. Minimum 5 years of nursing experience (nursing home/ILTC or community care setting are preferred)
. Strong clinical assessment and care planning skills
. Knowledge of gerontological nursing and chronic disease management
. Effective communication with residents, families, and multidisciplinary teams
. Proficient in basic computer skills
. Brings strong knowledge of workplace learning and people development trends, with a proactive and positive mindset.
. Is independent and resourceful, with a strong understanding of organizational development needs. Meticulous, well-organized, and an effective communicator.
. Skilled in stakeholder management and thrives as a collaborative team player.
