As an Assistant Nurse Clinician (Home Care), you will be responsible for the following duties:
- Provide clinical leadership, mentorship, and guidance to nursing staff to ensure the delivery of safe, evidence-based, and high-quality patient care.
- Assist in triaging patient referrals and contribute to case reviews to support timely care coordination and planning.
- Participate in departmental and multidisciplinary meetings, while supporting daily operational and administrative functions.
- Manage and provide support for clinical issues escalated by nursing staff, offering timely advice and clinical direction.
- Oversee nursing practices, staffing allocation, and resource utilisation, and recommend improvements to enhance operational efficiency and patient care.
- Contribute to the development, review, and implementation of community nursing policies, clinical guidelines, and standard operating procedures.
- Support staff development through performance coaching, competency assessment, and the identification of training and professional development needs.
- Take part in quality improvement projects, clinical audits, and service enhancement initiatives to promote patient safety and continuous improvement.
- Perform other clinical, operational, and administrative duties as assigned by the reporting manager.
Requirements:
- Possess at least an Advanced Diploma in Nursing (any discipline).
- Registered Nurse with at least 7 years of clinical nursing experience in an acute hospital and/or community care setting.
