Staff Nurse (Senior Care Centre)

Key Responsibilities

Clinical Assessment & Care Planning
. Conduct comprehensive admission and periodic assessments using standardized tools (e.g., Modified FRAT, FAST).
. Develop, implement, and update individualized care plans via the Elder &Community Care suite.
. Perform critical reassessments following hospitalizations, falls, or significant absences.

Medication & Clinical Management
. Oversee medication management, including preparation, packing, and administration.
. Ensure strict adherence to the IDDSI framework for residents with swallowing difficulties (dysphagia).
. Respond promptly to medical emergencies and maintain rigorous infection control standards.

Leadership & Supervision
. Supervise and train care staff in delegated nursing procedures, enteral feeding, and medication support.
. Liaise with the Centre Lead regarding nursing equipment and day-care operations.
. Lead quality improvement initiatives to enhance resident safety and well-being.

Coordination & Communication
. Collaborate with a multidisciplinary team to ensure seamless care coordination.
. Maintain effective clinical communication with families regarding client conditions and care goals.
. Facilitate internal and external referrals to support holistic resident needs.
