General Job Description
To assist in the planning, execution, monitoring, and evaluation of programmes and in the smooth running of daily operations of the Assistance & Well-Being (A&W) service function in ACMI.
Provide community-based case management services and support to ACMI's clients (individuals and families), assessing their needs, and developing personalised plans to support their well-being.
Assist in compiling relevant reports of programmes and activities for management decision-making.
Key Responsibilities:
Case Management Services
- Conduct assessments, develop case management plans, and implement interventions to address, guide and support clients needs and goals.
- Provide ongoing support and guidance to clients, monitor progress and adjust plans as needed.
- Provide crisis intervention in cases as needed.
- Collaborate with other professionals, such as social service agencies, organizations, healthcare providers, embassies, and community partners, to ensure comprehensive care and well-being.
- Facilitate access to resources, services, and benefits (e.g. financial assistance, housing, employment support) for clients.
- Maintain accurate and up-to-date records of client interactions, assessments, and progress.
- Participate in team meetings, case conferences, and training sessions to share knowledge and best practices.
- Travel out of office when needed for home visits, case conferences, and other necessary activities for case management.
Programme and service planning
- Assist in the development and monitoring of A&W programmes /services / projects.
- Assist in the smooth running of daily operations of the ACMI Helplines and Operating Centres.
- Assist in preparing grant applications and planning proposals for new programmes / services / projects in A&W.
- Develop and maintain relationships with community groups,organisations, and stakeholders to enhance service delivery and outcomes.
- Stay updated on industry trends, research, and policy developments to enhance service delivery.
- Ensure that ACMI's Philosophy, Vision and Mission are integrated and adhered to in all aspects of work.
Volunteer Development
- Assist in identifying criteria for and responsibilities of volunteers for A&W
- Assist in on-boarding volunteers assigned to A&W
Administrative Duties
- Ensure all relevant working processes, including the overall progression of projects/program and case updates are properly documented.
- Assist in preparing and monitoring A&W's yearly budget.
- Ensure proper accounting records to safeguard against misappropriation of the organisation's assets.
- Undertake any other duties as required.
Qualifications, Educational Requirements and Required Experience
- Minimum a recognized Diploma in Social Work, Psychology, Sociology, or a related field.
- Minimum 3 years of experience in case management, social work, or a related field.
- Experience in working with vulnerable populations (e.g., low-income families, migrants) is desirable.
- Experience in working with diverse client groups of different nationalities and cultures, including children, adults, and families is desirable.
- Have applied social work/psychology/counselling principles, theories, and practices in previous work experience.
