6 Months Contract (Maternity Cover)
Responsibilities:
Provide bio-psychosocial and financial assessment, social work intervention. These include the following:
1. Ensure comprehensive documentation where all relevant information related to client care is recorded and available to all members of the multi-disciplinary team.
2. Follow up on case management work and administrative work regularly.
3. Execute relevant psychosocial interventions to improve the well-being of clients.
4. Counsel and conduct financial assessments of clients and their families, and recommend appropriate interventions to support holistic client management.
5. Execute individual care plan in consultation with the clients and multi-disciplinary team, implement care plan, and evaluate progress.
6. Perform field work involving home and work site visits and visit to relevant social agencies.
7. Conduct group programmes (e.g., Caregivers support groups) to educate, equip, engage, and empower caregivers providing training and emotional support.
8. Lead in multi-disciplinary team meetings and facilitate case conferences where necessary.
9. Performs and network with relevant organizations, volunteers, social service agencies and individuals to activate social and financial resources to meet clients needs.
Return-to-Work Coordination
Coordinate and support clients on ABLE's Return-to-Work programme by reintegrating them into suitable and sustainable employment:
1. Provide vocational support to guide clients in goal setting and actualization.
2. Liaise between healthcare providers, client and potential employers to facilitate clients early return to work, including addressing the need for light duties or accommodations where relevant.
3. Provide pre- and post-placement support to enable clients to settle in and sustain employment.
