Assess clients during admission and review their physical condition regularly.

Develop and update individual care and rehabilitation plans.

Maintain accurate records of client assessments and treatment progress.

Work with the multidisciplinary team to discuss and coordinate client care.

Coordinate and supervise community outreach and therapeutic programmes.

Monitor service quality and clinical performance to support continuous improvement.

Collect and maintain information for regular reports.

Maintain good relationships with clients and provide quality customer service.
