Job Description

  • Assess patients physical conditions and functional abilities using appropriate assessment methods.

  • Formulate personalised physiotherapy care plans with measurable treatment goals.

  • Deliver home-based physiotherapy services to support patients rehabilitation and functional recovery.

  • Provide caregiver education and recommend appropriate assistive devices.

  • Support discharge planning and coordinate referrals to other healthcare services.

  • Participate in monthly home therapy case reviews and team meetings.

  • Facilitate therapy in-service sessions and participate in departmental training programmes.

  • Maintain clinical documentation and perform related administrative duties.
