Senior Manager/Manager, PHIC ICCO

The Senior Manager/Manager provides strategic leadership and operational oversight for teams within Integrated Continuing Care Operations (ICCO), driving healthcare initiatives that enhance care coordination between acute and community settings. This role include:

• Provide strategic direction and oversight for Integrated Continuing Care Operations (ICCO) teams, including Tiong Bahru Community Health Centre (TBCHC) operations and the EAGLEcare programme, ensuring alignment with organisational objectives and population health goals.
• Lead engagement of internal and external stakeholders and oversee team management of projects and resources to achieve desired project outcomes.
• Conduct comprehensive assessments of nursing home capabilities and stakeholder readiness to identify critical requirements for successful care integration initiatives, ensuring sustainable and effective implementation of collaborative care models.
• Manage and support programme implementation process – programme scoping, analysis, problem solving, implementation, risk management, resource utilization, communication and project accounting.
• Ensuring operational excellence at Tiong Bahru CHC by maintaining high-quality community health service delivery standards and achieving operational efficiency targets and performance indicators.
• Ensuring operational excellence for the EAGLEcare programme, including oversight of service delivery standards, resource allocation, performance monitoring, and continuous improvement initiatives to optimise programme effectiveness and programme outcomes.
• Work with Finance to raise and process claims with the necessary documentation in accordance with the financial guidelines, processes and governance rules.
• Maintain necessary documentation for audits and serve as the main contact point for audit queries.
• Assist to resolve and follow up on escalation cases of patient feedback and enquiries.
• Develop and implement appropriate workflow to ensure statistical reports are generated and reported to relevant stakeholders.
• Manage the annual workplan exercise and review department's yearly OPEX.
• Maintain and oversee contractual agreements with external stakeholders, ensuring compliance, performance standards, and regulatory requirements whilst fostering strategic partnerships that support integrated care delivery and operational sustainability.
• Engage proactively with diverse internal stakeholders across various specialties and external partners, including nursing homes and SingHealth Office of Regional Health etc., to effectively communicate operational changes, facilitate collaborative improvements, and ensure seamless implementation of care integration initiatives that enhance population health outcomes.

Requirements:

• Bachelor Degree with at least 6 years relevant experience in healthcare.
• Possess strong oral and written communication skills
• Strong analytical skills with an understanding of financials, statistics and workflow engineering and re-engineering would be an added advantage
• Independent and yet effective in a team setting
• Proactive and resourceful
• Meticulous with an eye for detail
• Able to demonstrate critical thinking through the execution of job tasks
