Senior Case Worker, Sanctuary Care

Job Description

Job Purpose

To effectively supervise case workers and interns to deliver good results in case management, programmes, community outreach, respite carers recruitment and publicity including assist Director (Clinical Services) in the running of the operations of Sanctuary Care Management.

Responsibilities

  • Spearhead in case movement through regular team meetings, case review session etc.
  • Identify at-risk situations, develop strategies and intervention plan to address child safety issues, and wellbeing.
  • Monitor and evaluate effectiveness of case management and other support programme and activities extended to respite carer families.
  • Development of relevant SOPs for the department.
  • Conduct in-depth assessment and produce written report on the suitability of applicants for the role as respite carer, present findings to the management of Boys Town and follow up on decisions and recommendations.
  • Match respite families with a foster child, based on factors such as the CANs assessment and/or age race religion as well as the foster parent's capability in meeting the child's needs.
  • Conduct all the necessary checks including announced and unannounced home visits to ensure that the respite carer is able to safeguard and promote child's safety, stability and wellbeing.
  • Organise and participate in inter-agency efforts in support the welfare and development of the foster child/CYP.
  • Coordinate a 24 hour hotline for respite carer per duty schedule and ensure that the team follows up on outcomes.
  • Supervise and support foster placement so as to prevent breakdowns in foster child's existing care arrangements.
  • Produce accurate recordings on the cases and to submit reports punctually, to Boys Town and other partners.
  • Participates in road shows and other community outreach events as a when necessary.
  • Meet regularly with Head Sanctuary Care/supervisor to assess case progress and programme outcome/effectiveness.
  • Manage and support volunteers interested in providing their service to Sanctuary Care.
  • Contribute relevant articles and social media content to promote mental health.
  • Coordinate some marketing and publicity projects to promote Sanctuary Care services.
  • To support and conduct research related to practice.

Requirements

  • Minimum 5 years of relevant experience supervising.
  • Experience in working with children/CYP and their families.
