Job Description

Brief Summary

Join a dedicated team as an Occupational Therapist, where you'll provide high-quality, client-centered therapy services while collaborating with a multidisciplinary group to enhance client outcomes.

Responsibilities

  • Conduct comprehensive assessments for clients during admissions, periodic reviews, and discharges to inform therapy plans. Identify and implement suitable outcome measures for timely evaluation of therapy progress and decision-making. Collaborate with clients and key stakeholders to establish SMART, person-centered goals. Develop and execute evidence-based occupational therapy interventions tailored to individual client needs. Maintain accurate documentation of assessments, interventions, goals, and outcome measures in compliance with guidelines. Communicate effectively with clients and stakeholders regarding assessment results, intervention plans, and discharge strategies. Ensure proper utilization of equipment, including training clients in the use of assistive devices. Recommend appropriate assistive devices and equipment based on client needs. Create discharge and continuity of care plans in consultation with relevant parties to ensure ongoing support. Foster community integration by collaborating with external partners as needed. Empower clients and caregivers through strategies that enhance health, mobility, and overall function. Participate in multidisciplinary case discussions to ensure a collaborative approach to client care. Oversee accurate and timely documentation related to client care and outcomes. Assist in the development and review of clinical programs and services, supporting training and supervision of therapy assistants. Engage in continuous professional development and research initiatives to enhance service quality. Adhere to audit requirements and undertake additional duties as assigned.

Requirements

  • Minimum a Diploma or Degree in Occupational Therapy, fully registered with AHPC. At least 2 years of relevant experience, preferably in neuro-rehabilitation. Strong interest and knowledge in neurological, geriatric rehabilitation, and musculoskeletal conditions. Dynamic and proactive personality with strong public relations and communication skills. Well-developed management, analytical, and problem-solving capabilities.
