- Provide high-quality, professional physiotherapy services across assigned caseloads, including inpatient, outpatient, and home-based cases.
- Ensure service coverage for designated wards.
- Conduct and document all aspects of patient care, including initial assessments, periodic reassessments (every 6 months for maintenance and every 3 months for active rehabilitation), individualized rehabilitation goals, weekly review of rehabilitation plans, treatment updates, and discharge planning.
- Monitor and follow up on patient progress.
- Participate in case conferences, multidisciplinary team meetings, and family meetings.
- Implement appropriate safety measures to protect patients during rehabilitation, including conducting risk assessments of the clinical environment.
- Communicate effectively with patients and their families to support rehabilitation, reinforce skill application, and promote therapeutic carryover beyond therapy sessions.
- Ensure the safe use of all equipment and promptly report any defects, incidents, or complaints to the appropriate authority for resolution.
- Actively engage clients, families, and caregivers in treatment planning, reviews, and rehabilitation processes.
- Provide safe and professional caregiver training to family members or carers when required.
- Utilize evidence-based, standardized assessments and interventions to assess, plan, implement, and evaluate care.
- Monitor, evaluate, and review Occupational Therapy treatments within a multidisciplinary framework, making necessary adjustments to optimize outcomes.
Requirements:
- Master/Bachelor/Diploma in Occupational Therapist, recognized by the Singapore Allied Health Professional Council (AHPC)
- Registered with AHPC and holding valid practicing certificate from AHPC
- BCLS + AED Certificate
- 2-3 years experience relevant experience in hospital or nursing home setting
5 days work week Working hours: 8.30AM to 6PM
