Job Description: CAREwell Key Worker

About CaringSG Limited
CaringSG Limited is a charity led by caregivers, serving special needs caregivers and the wider community to build an inclusive and caring Singapore. Since our inception, we have successfully garnered strong stakeholder support and demonstrated significant cross-sectoral impact through the implementation of Project 3i.

We are seeking a CAREwell Key Worker to join our team and contribute to CaringSG's growth and the success of the CAREwell programme.

Role Overview

The CAREwell Key Worker will work closely with the CAREwell team to provide direct support to clients of the CAREwell (CW) programme and assist in programme development. The role requires expertise in service coordination and case management, including end-to-end engagement with clients and stakeholders, case assessment, goal-setting, care plan design and implementation, review, and documentation.

Key Responsibilities

  • Conduct needs assessments for clients, dependents, and families (online and onsite).

  • Perform clinical case formulation, develop SMART goals, and implement care plans systematically.

  • Coordinate services and make referrals to external agencies.

  • Provide clients with relevant information and resources.

  • Maintain accurate records and prepare reports.

  • Participate in training, supervision, and team meetings.

  • Remain vigilant for potential harm to clients or others (e.g., child abuse, domestic violence, substance abuse).

  • Report disclosures of harm to self or others to the supervisor and senior management and handle matters appropriately.

Candidate Requirements

Technical Competencies

  • Strong interest and prior experience working with special needs families, children, persons with disabilities, and community stakeholders (e.g., family service centres, special schools).

  • Strong interpersonal and communication skills.

  • Competence in para-counselling micro-skills.

  • Understanding of the bio-psycho-social-spiritual model, family-centred practices, and trauma-informed care principles.

  • Ability to manage crisis intervention independently and collaboratively.

  • Creative, resourceful, and willing to learn from other disciplines.

  • Capable of role release in a transdisciplinary team.

  • Strong organisational and multitasking skills.

  • Able to make timely and well-paced decisions for optimal outcomes.

  • Meticulous, with good awareness of professional boundaries, client confidentiality, and risk management.

  • Confident in making presentations and engaging stakeholders or collaborating agencies.

  • Proficient in computer use for various tasks.

  • Skilled in Microsoft Office (Word, Excel, OneDrive, PowerPoint) and Google Suite (Forms, Docs, Sheets, Drive).

Preferred Characteristics

  • Singapore Citizen or Permanent Resident.

Work Arrangement

  • Flexible: combination of online and onsite interviews, consultations, discussions, home visits, service coordination, and inter-agency case conferences, including after office hours where necessary

  • Weekly in person team meetings.

Employment Details

  • Experience At least 3 years experience working with clients and their families affected by developmental and behavioural challenges, disability, neglect, child abuse, domestic violence, mental health issues, or family support needs.

Salary Range:

  • $4,000 - $5,000 (Full-Time, Contract Basis)
