Responsibilities

• Conduct comprehensive biopsychosocial and environmental assessments to identify client needs. Develop and continuously evaluate nursing care plans to adapt to client's changing requirements. Build therapeutic relationships that empower clients, families, and caregivers for effective self-management. Coordinate with multidisciplinary teams to enhance care integration and assist clients in navigating the healthcare system. Manage timely planning and scheduling of care interventions, including wound care and medication management. Supervise and guide Enrolled Nurses and nursing students during their clinical attachments in achieving learning objectives. Ensure compliance with safety, infection control measures, and confidentiality regulations.

Requirements

• Registered Nurse with a valid license from the Singapore Nursing Board. Minimum of 3 years of experience in acute and/or community healthcare settings. Diploma in Nursing is required. Proficient communication skills to establish and maintain therapeutic relationships for optimal patient care. Ability to make informed clinical judgments regarding patient care independently.
