Job Summary
Lead and deliver high-quality, home-based nursing care for clients with complex medical needs. Combine clinical care with team leadership and care coordination to ensure safe, person-centred services and mentor nursing staff.
Responsibilities
Clinical Care & Service Delivery
- Provide direct nursing care to clients in home-based settings to meet their medical and functional needs
- Support care planning and clinical decision-making for clients with complex health conditions
- Monitor and evaluate care outcomes to ensure safety, quality, and continuity of care
- Maintain accurate, compliant clinical documentation for all care activities
Team Leadership & Supervision
- Supervise nursing staff and oversee daily operational activities to ensure effective service delivery
- Provide hands-on guidance, coaching, and mentorship to nursing team members to enhance their skills
- Assist in onboarding, training, and developing nursing staff competencies for improved performance
- Identify and escalate clinical risks and operational issues promptly to appropriate stakeholders
Care Coordination & Partnerships
- Collaborate with multidisciplinary teams and community partners to support integrated care delivery
- Participate actively in case discussions and contribute to comprehensive care planning
- Facilitate smooth care transitions and continuity across different healthcare services
Quality, Safety & Governance
- Support audits, quality improvement initiatives, and service evaluations to enhance care standards
- Manage incident reporting processes and follow up on corrective actions to mitigate risks
- Ensure compliance with clinical protocols and regulatory standards to maintain service integrity
Required competencies and certifications
- Bachelor's Degree in Nursing or equivalent
- Registered Nurse with Singapore Nursing Board (SNB)
Preferred competencies and qualifications
- 4-6 years of nursing experience, preferably in community or eldercare settings
- Prior supervisory or senior staff nurse experience
