To receive referrals from referral source(s) and prepare them for screening by the Rehab Lead/ Centre Lead/Nurse
To schedule appointments for initial assessment, financial counselling and regular therapy sessions
To perform counselling explaining to clients and/ or their next of kin regarding the contract of service
To conduct means testing for patients if required
To prepare and maintain order of case notes for new admissions and for scheduled therapy sessions
To process admission and discharge of clients
To enter client's attendance into the EMR daily and update the system as necessary
To liaise with Operations & Facilities Management (OFM) personnel for transport arrangement for clients if required
To liaise with the Business Office or Finance personnel for billing matters
To update the Agency for Integrated Care (AIC) website EMR and Transport Call Centre (TCC) system as and when necessary
To perform reception duties, including responding to phone calls and attending to general enquiries
Carry out other tasks/ projects assigned by the Centre Lead
To communicate to clients/ next of kin and staff in a professional manner
Communication should reflect consideration of socio-cultural values of the patient, family and significant others
To maintain absolute confidentiality in matters pertaining to the organization/departmental/clients. Confidentiality of the clients records is to be always maintained to maintain privacy of the clients
Takes responsibility for own learning
Participates in staff meetings and hospital initiatives
To liaise with external services, statutory bodies and other VWOs as appropriate
Job Requirements
Certificate in Healthcare Support (Administration) or equivalent is desirable
Working in a geriatric set up, long term care or nursing home set up will be an advantage but not necessary
