Responsibilities

Make home visit to patient home
Provide chronic disease, activities of daily living and/or palliative advice and counselling, as well as nursing procedures such as nasogastric tube insertion, catheter management and wound care.
Write case report and update sytem on patient condition
Liaise with relevant personnels and tap into resources to provide for patients needs
Provide emotional support for patients and family members

Job Requirements

Min. Diploma/ Degree in Nursing
Valid SNB/BCLS
Min. 3-5 years of relevant perm acute wards / hospice experience in gerontology and / or palliative care preferred
