Job Summary
Metta Home for the Disabled provides long-term residential care for adults with intellectual and multiple disabilities. As a Social Work Assistant, you will support Social Workers and the multidisciplinary team to enhance residents well-being, coordinate programmes, and provide administrative support for smooth social work service delivery.
Responsibilities
- Case Management & Client Care
- Conduct client assessments and manage admissions, care planning, reviews, and discharges.
- Monitor clients needs and coordinate care with the multidisciplinary team.
- Conduct home or hospital visits when required.
- Caregiver Support
- Provide counselling, emotional support, and caregiver education.
- Assist with Means Testing, financial assistance applications, and access to community resources.
- Multidisciplinary Collaboration
- Work closely with healthcare professionals and participate in case discussions and service planning to ensure holistic care.
- Documentation & Reporting
- Maintain accurate client records, reports, and database updates.
- Prepare and submit subsidy claims, monthly reports, and required statistics.
- Ensure compliance with organizational policies and regulatory requirements.
- Other Duties
- Supervise and guide Social Work Administration staff.
- Support audits, organizational projects, and events.
- Perform other duties assigned by management.
