RESPONSIBILITIES

The Social Worker (Community Case Management Service - CCMS) provides professional case management services to vulnerable seniors and their caregivers with health and social care needs. The role involves the application of foundational social work skills to conduct biopsychosocial and functional assessments (including interRAI Home Care), develop and implement individualised care plans under supervision, and deliver ongoing casework interventions to support clients safety, well-being and ability to age in place.

The Social Worker works collaboratively with healthcare providers, social service agencies and community partners to support care coordination and service integration, and to assist in the management of complex and high-risk cases in accordance with organisational protocols and supervisory guidance. The role contributes to programme outcomes by reducing avoidable hospital admissions and premature institutionalisation, and by ensuring clients receive appropriate, timely and coordinated care in the community.

Clinical & Case Management

  • Conduct biopsychosocial and functional assessments for seniors with health and social care needs using structured assessment tools (including interRAI Home Care) under supervision
  • Develop and implement individualised care plans with guidance from supervisors, in collaboration with clients and caregivers
  • Provide ongoing case management by monitoring client progress and implementing planned interventions under supervision
  • Establish rapport and maintain professional relationship with client and significant others
  • Support the management of complex or high-risk cases (e.g. safety concerns, caregiver stress) in accordance with organisational protocols under supervisory guidance
  • Prepare social reports and maintain accurate, timely case documentation and records

Care Coordination & System Navigation

  • Coordinate and follow up on health and social care services for clients under guidance
  • Participate in case conferences and multidisciplinary discussions as a team member.
  • Facilitate referrals to appropriate community resources and financial assistance schemes with supervisory support
  • Support safe discharge, step-down care and case closure when sustainable care arrangements are established

Community Engagement

  • Assist in community development initiatives that strengthen support networks for seniors and caregivers
  • Collaborate with partner agencies to support service delivery and address identified client needs

Professional Practice & Service Development

  • Participate actively in supervision, training and reflective practice to develop professional competencies
  • Assist in service evaluation and quality improvement activities as part of the team
  • Uphold professional ethics, organisational policies and regulatory requirements in all aspects of work
QUALIFICATIONS
  • Possess Degree in Social Work or Degree with a Graduate Diploma in Social Work or a relevant degree in social sciences or related discipline(s) (e.g. psychology) from an accredited institution.
OTHER INFORMATION

Relevant Experience:

  • Prior experience as a social worker would be an advantage
  • Accredited as Registered Social Worker with Singapore Association of Social Workers would be an advantage
  • Fresh graduates are welcome to apply
