Location: Woodlands/Buangkok/Bukit Batok

Main Duties and Responsibilities

Daily Duties

. Develop individualise care plans for residents.

. Execute the care plan and monitor progress.

. Coordinate care for residents with relevant stakeholder.

Requirements

. Degree in Social Work, Psychology or Counselling, or a Graduate Diploma in Social Work, Psychology or Counselling.

. Willing to comply with the Standard Operating Procedure when discharging duties.

. Willing to perform shift work in accordance with the Duty Roster.

. At least 1 year of relevant experience.
