Social Worker/ Case Worker

Available locations: Woodlands/Buangkok/Bukit Batok
44 hours work week, Perm Position

Responsibilities

• Develop individualise care plans for residents
• Execute the care plan and monitor progress
• Develop and implement programmes and activities to meet the residents care needs

Requirements

• Degree in Social Work or Psychology
• Willing to comply with the Standard Operating Procedure when discharging duties
• Willing to perform shift work in accordance with the Duty Roster
• At least 1 year of relevant experience
