Job Responsibilities:
- Assessment and Counseling:Conduct thorough assessments of clients through home visits or social visits, evaluating their social and emotional needs.
Provide counseling and assess the financial condition of clients to understand their support systems and challenges.
Recommend appropriate solutions and develop individualized plans to address patients needs effectively.
- Coordination and Advocacy:Coordinate with various parties, including community organizations, government agencies, and individuals, to advocate for clients benefits and welfare.
Act as a liaison between patients and support services, ensuring they receive necessary assistance and resources.
- Education and Support:Educate patients and families about illnesses, treatment plans, and available resources to empower them in managing their health effectively.
Conduct psychosocial assessments to identify mental or emotional distress and provide crisis intervention and support as needed.
- Comprehensive Care Planning:Assess each patient's unique condition, combining medical diagnoses and personal interviews to develop comprehensive care plans.
Provide necessary support tailored to each patient's health status, personal strengths and weaknesses, and emotional state.
- Resource Evaluation and Follow-Up:Research local support groups and resources, matching them to individual patients needs and preferences.
Evaluate the effectiveness of services and resources provided to patients and adjust plans accordingly.
Follow up with patients throughout their diagnosis, treatment, and recovery process to assess their mental and emotional well-being and provide ongoing support.
