Social Worker / Case Management

• Casework management and support with biopsychosocial-spiritual assessment & intervention of patients.
• Collaborate with multi-disciplinary team to refer patients to suitable the programmes for financial assistance.
• Participate or assist in developing, conducting and evaluating patients support groups and community outreach programmes.
• Deliver psychoeducation to patients, families, staff and members of public through workshops & seminars.
• Participate in organization/department initiatives and projects.

Requirements:

• Degree in Social Work/ Post Graduate Diploma in Social Work with2 years of social work experience
• Candidates with aPost-Graduate Diploma in Counsellingwill have an added advantage
• Effective clinical skills in assessment, intervention, casework and counselling
