Assess, coordinate and facilitate new referrals in a timely manner.
Create and implement discharge plans to guide clients and their caregivers.
Evaluate programme to meet clients needs by monitoring and reviewing clients progress and outcomes.
Develop individual care plans, ensure they're implemented, and carry out regular review of the cases.
Identify, assess, facilitate, advocate and monitor clients psycho-social needs, as part of a transdisciplinary care plan.
To provide support and empower clients families / caregivers, by helping them to assess and link to reliable social support system, through providing information and referral services.
Responsible for the bi-yearly review of means-testing for clients, and to advise clients and their caregivers 3 months before their Household Means Test expires so that it can be renewed.
To refer suitable clients for Medifund assistance, prepare the documentation and present their cases to Medifund committee, answer questions related to the cases.
Maintain and update records / case files of clients for administrative, intervention and audit purposes.
To guide remaining Carer's Support clients and prepare them for discharge either by referring them to other agencies or other means.
Support the social work needs for 2 centres.
Other Responsibilities

To record, collate and provide relevant statistics required by stake-holders.
