Job Description

As Social Workers at Autism Association (Singapore) (AA(S)), we advocate for the needs of individuals on the autism spectrum, facilitate their access to resources and betheir connecting bridge between AA(S) services, family and the community. Our mission is to enable the individuals to maximise their potential, helping them lead meaningful and quality lives in society.

We are looking for individuals who are dynamic, collaborative and possesses high level initiative with a passion to serve the autism community to join our team.

DUTIES AND RESPONSIBILITIES

1. Targeted Case Management

. Provide end-to-end case management, intervention and evaluation for students flagged up for attendance issues, behavioural concerns or other critical needs

. Collaborate and coordinate multi-disciplinary case discussions with relevant departments, family and community partners for students/clients assessed for higher support needs

. Formulate detailed reports on a students social and family background, including financial needs and support systems to aid in the school's holistic student developmental planning and reporting to government agencies (e.g. Child Protective Service(CPS))

. Consistently connect with families at students key transition points (including orientation, progress meetings, transition planning discussions and graduation ceremony) to foster close school-family collaboration

. Utilise different modes of engagement (e.g. home visits, tele-conference sessions) for the purpose of assessment, intervention and monitoring of case needs

. Assess referrals for child protection concerns using CPS Child Abuse Reporting Guide (CARG) and Sector-Specific Screening Guide (SSSG) and act as the liaison between school and CPS

. Maintain updated documentation of sessions with families and meetings with stakeholders using digital platforms

2. Post-School Transition Support

. Facilitate person-centred planning meetings that bring together the student and significant persons in the student's life to collaboratively plan for the student's future

. Consistently collaborate with parents, educators and Transition Planning (TP) team on students post-school transition planning process

. Complete social reports to facilitate graduating students applications to Government-funded post-school services

. Act as the Point-of-Contact(POC) between families and SG Enable for students post-school application matters

. Be the key resource point for families on information on post-school options in the community

. Conduct periodic check-ins for graduates post-graduation status

3. Administration of school-related subsidies

. Conduct financial assessments and make recommendations on appeals for additional subsidies

. Ensure compliance to government's subsidy claims submission deadlines

. Collaborate with relevant school departments for the administration and review of subsidy redemption processes

. Act as the POC between families and government agencies for students subsidy-related matters

. Support management in the preparation of annual subsidy audits

4. Donation management

. Assess and make recommendations for students eligibility for donations/grants

. Facilitate applications for community-based grants for students

. Act as the POC between donors and students

. Utilise e-system to record and track students donations/grants received

5. Professional Development and Education

. Maintain professional growth and development through participation in training programmes, supervision,conferences and other community-based learning opportunities

. Sharing with other Social Workers and other relevant school departments on relevant organisation/community-wide knowledge and processes (e.g. Child Protection reporting guide)

6. Other duties asassigned by management

. Collaborate with other school departments in planning, implementing and evaluating organisation-wide events (e.g. Staff Appreciation, The Purple Parade)

. Perform any other relevant duties as requested by the management

Key requirements:

  • Degree or Graduate Diploma in Social Work
  • At least 2 years of experience in case management in the social service sector
  • Strong interpersonal and communication skills
  • Collaborative and effective team-player
  • Proactive and self-motivated towards continuous learning, including in use of digital processes
