A.Administrative and Scheme-Related Support
- Referral Intake and Screening, collaborate with Registered Nurse to conduct intake screening for resident.
- Complete social assessment for general understanding of family situation
- Pick up cases which require further psychosocial-spiritual support
- Admission
- Touch base with new resident to check in on transition coping
- Respite Care / Receive urgent / planned respite cases
- Work with team to ensure admission and work with relevant agencies and families on long term plan e.g. permanent placement in NH or discharge arrangements
- Assist residents in applying for government financial assistance programmes, including:
- Interim Disability Assistance Programme for the Elderly (IDAPE)
- Eldershield Claims
- Medical Fee Exemption Card (MFEC)
- Medifund
- ElderFund
- CareShield Life / MediShield Life
- Home Caregiving Grant
- PGDAS
- Silver Support Scheme
- Other financial schemes as applicable
B. Psychosocial Support and Casework
- Conduct intake screenings jointly with the Registered Nurse, and complete social assessments to identify clients requiring further intervention.
- Provide emotional support to residents and their families.
- Manage day-to-day casework, including coordinating referrals and external resources.
- Facilitate group activities, festive celebrations, and outings to support social engagement.
- Plan and conduct Household Committee meetings to check in on residents well-being.
- Engage new residents and families to support their transition into the facility.
C. Advanced Care Planning and End-of-Life Support
- Ensure clients have completed Advance Care Planning (ACP) and/or Preferred Plan of Care (PPC) where applicable.
- Initiate conversations on end-of-life preferences, including life reviews, final wishes, and funeral arrangements.
- Provide bereavement support and short-term counselling to grieving families refer for further support as needed.
- Coordinate pro bono funeral arrangements for residents without next-of-kin (NOK).
D. Intervention
- Work closely with internal teams and external agencies to implement appropriate intervention plans.
E . General Duties
- Keep up to date with government schemes, social policies, and referral services to support clients effectively.
- Attend and contribute to multidisciplinary team meetings.
- Handle walk-in and public enquiries.
- Maintain accurate documentation in accordance with PDPA and organisational standards.
- Contribute to process improvements and team-based initiatives.
- Perform other related duties as assigned.
Qualification & Experience
- Diploma in Social Sciences
- At least 2 years of experience in Healthcare / ILTC social work settings
- Ability to handle sensitive and confidential information with discretion
