You will provide support to the Medical Social Workers in obtaining patients social, familial and financial information to facilitate appropriate service referrals. You will conduct home visits and interviews to ensure that patients receive continuity of care upon their discharge as well as to determine the psychosocial adjustment of patients in the community.
Requirements
- GCE O/A level with at least 2 years relevant experience in social services; or Diploma preferably in Community Services or Human Services
- Good team player with passion in working with people
- Excellent communication skills
- Proficient in Microsoft Office applications
