Job Summary
Purpose of job:
- execution of casework support under the guidance of a social worker, including home visits to engage clients and caregivers living with mental health conditions.
- Support basic intervention tasks identified by social workers.
- Engagement of clients with dementia with meaningful activities through befriending and cognitive stimulation programmes.
- Monitoring of client's conditions and progress.
- Provide service linkage for client and caregivers.
- Support the team by providing administrative, operational and logistical assistance for client-related services, programmes, outreach efforts and group work activities.
Key Responsibility Areas
- Passion to work with individuals and caregivers with mental health conditions.
- Basic befriending, , elder sitting duties and emotional support for elderly.
- Support seniors and caregivers during service duration.
- Perform basic mental health screening tools for clients and caregivers.
- Support community outreach, group work and referrals.
- Ensure proper documentation of case notes, assessment, records.
Requirements
Experience:
- Prior experience in working with elderly clients and caregivers in the community setting.
- Relevant working experience in healthcare or social service sector.
- Able to co-facilitate group work.
Qualifications:
Professional Cert/Nitec/Diploma
Others:
Requires travel and home visits to various locations within the service area.
