Job Description

1. Patient Onboarding & Education – Support patients in joining and understanding the Smart Triage and TEMPO programmes.

• Assist with patient onboarding into the Smart Triage and TEMPO programmes.

• Explain programme objectives, benefits, workflows and patient responsibilities.

• Educate patients on the proper use of medical devices, including blood pressure monitors and pulse oximeters.

2. Digital Health Support – Help patients use the relevant digital platforms accurately and independently.

• Guide patients in navigating digital platforms such as FormSG or MyChart.

• Support patients in submitting vital signs and symptom reports accurately and independently.

3. Patient Engagement – Engage patients to support participation and adherence to programme workflows.

• Engage patients through telephone calls or face-to-face interactions.

• Address patient queries and provide appropriate programme information where required.

• Encourage adherence to monitoring requirements and programme workflows.

4. Feedback Collection – Support the collection and maintenance of patient feedback.

• Administer feedback surveys to Smart Triage and TEMPO participants.

• Collect, compile and maintain accurate records of patient feedback.

5. Administrative & Operational Support – Provide project coordination, documentation and reporting support.

• Maintain accurate records of patient onboarding and engagement activities.

• Assist the project team with operational and administrative tasks related to programme implementation.

• Support data tracking, documentation and reporting requirements.

Role Requirements

• Meticulous, organised and attentive to detail

• Strong verbal and written communication skills

• Comfortable engaging patients by telephone and face to face

• Able to explain programme information and device use clearly

• Comfortable supporting patients with digital platforms

• Able to maintain accurate records and handle data responsibly

• Able to work collaboratively within a multidisciplinary project team

• Proficient in Microsoft Office applications
