Overall Job Responsibility

The Service Coordination and Advice team seeks to provide dedicated help to persons with disabilities and caregivers who require disability support and services, especially persons with disabilities who are not enrolled in any government funded disability services.

We hope to enhance clients service journey through person-centric support. We assess the needs of the families, coordinate case planning, provide information and advice as well as facilitate applications to suitable schemes or services in SG Enable or other agencies.

In this role, you will have the opportunity to improve service delivery to persons with disabilities and their families. You will lead in stakeholder engagements and/or training of frontline professionals to raise disability awareness, share resources and foster closer collaboration with SG Enable. You will work with the families to navigate suitable post-18 resources, help to continuously review and improve operations. Your good communication skills and ability to foster strong partnerships with relevant professionals will be instrumental to achieve our aim to better support clients.

Duties and Responsibilities

  • Oversee Adult Disability Referral Services
  • Lead the Adult Disability Referral (AD) team in enhancing client service journey through a person-centric approach, in case coordination, case planning and providing information and advice to families seeking Post-18 services
  • Lead AD team in the developing and implementation of referral workflows and processes, to ensure timely referrals
  • Support AD team leads in rendering support to cases officers in case coordination and case management (when required)
  • Oversee and guide AD colleagues in managing cases with complex needs, which may include working with multiple agencies to facilitate Whole-Of-Government approach to support clients
  • Continuously review Referral process and KPIs for the division
  • Facilitate workplan discussions with Team Leads for service improvement
  • Oversee and guide AD teams in managing referrals to non-mainstream services such as new pilot programmes (ESH, ESLP, ELP, DCMP, Transitional support, etc.)
  • Oversee and support AD teams for MP Appeals and engagement with community partners for case-related coordination
  • Stakeholder Engagement and Management
  • Develop and execute engagement plans with relevant stakeholders and MSF to review gaps and challenges, design and propose solutions to better serve persons with disabilities and their families
  • Oversee relationship management with multiple stakeholders which includes but not limited to Touch Points (SPED Schools, Government ministries, SSO, Healthcare, Community agencies and partners)
  • Contribute to SGE's efforts in Social-Health integration for disability sector
  • Partner with Goh Chok Tong Enable Fund (GCTEF) Business Unit to engage with community stakeholders for fund application and innovation
  • Oversee case referrals with AIC (aging clients), GCTEF (clients who require transitional support) and any other AD service collaboration related to SCA clients
  • Other Assignments
  • Devise annual work plans and budgets for the division with Senior Management
  • Oversee CAF-R Registry with SCA POC and MSF EN
  • Ensure timely and accurate review of Enabling Guide and Service Matrix with SCA POC
  • Any other duties as assigned

Qualifications, Knowledge And Experience

  • Relevant qualifications in social work, psychology will be an advantage.
  • At least 5 to 8 years of work experience in social service sector and preferably case management work.
  • At least 2 to 3 years of experience leading and supervising teams
  • Strong leadership skills and team player
  • Strong analytical skills
  • Able to independently engage stakeholders and foster strong partnerships
