The Enabled Living Programme (ELP) is a community living model which aims to enable
Persons with Disabilities (PwDs) to achieve a better quality of life through supported
community living. The Senior Training Officer will be responsible for the overall well-being,
safety, and care of ELP clients, while developing a strong support network through the
engagement of neighbours, community partners, family and caregivers, so as to empower
clients to live ordinary lives in ordinary homes in ordinary neighbourhoods.
Job Description
- Develop and implement Individual Client Plans (ICPs) collaboratively with clients and caregivers
- Support progress of clients according to their ICPs, utilizing positive behavior support and a person-centered approach
- Monitor progress of clients through the timely administration of assessment tools
- Maintain clients administrative records and ensure client information and plans are updated and current
- Provide teaching and support for clients in the development of independent living and social skills
- Plan and coordinate social engagements and inclusive activities for clients
- Build relationships and collaborate with neighbors and community partners to develop support networks for clients in the vicinity
- Collaborate with other colleagues in a multidisciplinary setting to provide learning, behavioral and socio-emotional support for clients
- Maintain effective and regular communication with family/caregivers
- Responsible for the overall well-being, safety, and care of clients
- Responsible for the upkeep of client residences and ensuring a safe and suitable living environment for clients
- Any other tasks as and when necessary
Job Specifications
- Minimum polytechnic diploma or equivalent, with at least 3 years of experience working with adults with mild intellectual disability
- Relevant experience in teaching/coaching adults with mild intellectual disability will be an added advantage
- Compassionate and committed towards serving adults with disabilities, and supporting them to live independently and meaningfully in the community
- Able to work shifts, including weekends and public holidays
- Adaptable and proactive towards managing an occasionally unpredictable workload
- Team player with good interpersonal skills, and an effective communicator
- Self-motivated and able to work independently
