Role: Paediatric Renal Transplant Nurse
Patient Population: Paediatric patients for renal transplantation
Specific Roles and Responsibilities
- Plan and conduct personalized patient and family education for pre and post renal transplant patients.
- Seve as a liaison person with adult transplant coordinator for paediatric renal transplant patients.
- Respond and address to immediate query/concerns that renal transplant patients or family may have related to child's condition and pre/post-transplant management.
- Assist in new and long-term post-transplant patients monitoring.
- Assist in kidney biopsies.
- Monitor primary patients progress and compliance to medication.
- Serve as resource person for renal transplant patients and families who require phone consultation service.
- Recommend appropriate referral to allied healthcare professionals.
- Plan and conduct staff education on paediatric renal transplant for nursing staff and clinical fellows/doctors.
- Serve as a resource to nursing staff, nursing students, and other healthcare personnel in acquisition of knowledge and skills related to renal transplant.
- Participate in the following:
Dialysis and Transplant clinic
Paediatric Renal Replacement Programme meeting
Annual Children's Kidney Centre camp
- Update and maintain transplant administrative activities.
- Document relevant registry statistics for transplant according to Ministry of Health requirements.
- Assist in the writing of transplant protocols.
- Develop, review and update teaching materials for patients / families with renal diseases.
- Coordinate activities and act as a link with established support groups like Club Rainbow for children and their families
- Provide coverage for urgent nursing duties under KTP NUCMI and take on additional responsibilities or tasks assigned by the senior nurse Manager/Clinician/Educator and Assistant Director of Nursing.
