Job Summary
Deliver specialized nursing care by collaborating with multidisciplinary teams to assess, monitor, and manage residents health conditions, ensuring safe medication administration and quality care in a nursing home environment.
Responsibilities
- Comply with approved nursing practice standards and professional guidelines to ensure safe and effective care delivery
- Apply specialized clinical nursing skills to provide comprehensive nursing services tailored to residents needs
- Collaborate with residents attending doctors and multidisciplinary teams to coordinate and optimize care plans
- Monitor, assess, and report timely changes in residents conditions, needs, and treatment responses to support clinical decisions
- Implement nursing care and interventions according to individualized care plans and evaluate their effectiveness
- Recommend and initiate revisions to care plans based on residents responses and clinical assessments
- Work with Allied Health Professionals to initiate preventive, rehabilitative, and restorative nursing interventions as needed
- Conduct and supervise comprehensive nursing assessments for new admissions to establish baseline care requirements
- Prepare, administer, and document medications accurately following safe medication practices and regulatory standards
- Ensure safe custody, administration, and documentation of controlled drugs in compliance with regulatory requirements
- Participate in training, supervision, and development of nursing and care staff to enhance team competencies
- Manage emergency situations by promptly mitigating risks and escalating issues to supervisors when necessary
- Collaborate with staff to maintain a safe, clean, and hygienic nursing home environment
- Participate in quality assurance activities and audits to monitor and improve nursing care quality and appropriateness
- Prepare duty rosters and assign daily staff duties to ensure adequate ward or department coverage
- Contribute to staff performance appraisals and evaluations in collaboration with the Nurse Clinician
- Act as deputy for the Nurse Clinician during their absence to maintain continuity of leadership
- Undertake additional duties as assigned by Senior/Nurse Clinician, Director of Nursing, or Chief Executive Officer
Required competencies and certifications
- Registered Nurse locally trained with Advanced Diploma, Diploma, or Bachelor's Degree in Nursing
- Foreign-trained Registered Nurse with Diploma or Degree in Nursing and recognized Board Pass (e.g., Professional Regulation Commission - PRC)
- Full registration with the Singapore Nursing Board and possession of a valid Practicing Certificate
