Senior/Staff Nurse (Home Care & Home Hospice)

Brief Summary

We are seeking a dedicated healthcare professional to deliver exceptional patient care, while collaborating with a multidisciplinary team to develop individualised care plans and improve healthcare services.

Responsibilities

• Conduct independent clinical assessments and formulate personalised care plans in consultation with the multidisciplinary team. Perform necessary clinical procedures to ensure optimal patient care. Provide on-call support during out-of-office hours, escalating urgent cases to the on-call physician as required. Assist in nursing consumables inventory management and tracking of controlled substances, while guiding junior nursing staff. Complete required competency training within designated timelines and document learning hours. Support clinical audits and supervision of junior nurses alongside the Nurse Clinician/Assistant Nurse Clinician. Actively participate in Quality Improvement (QI) and audit initiatives assume the role of assistant project manager when needed. Address any additional responsibilities related to healthcare services.

Requirements

• Registration with the Singapore Nursing Board. A minimum of 5 years of nursing experience, including at least 2 years in community healthcare settings. Preferred qualifications include a Specialist/Advanced Diploma in Gerontology, Palliative Care, or an equivalent field. Strong verbal and written communication skills. Ability to collaborate effectively within a team environment.
