As a Senior Staff Nurse (Home Care), you will be responsible for the following duties:

• Providing home visits to patient for symptom control and assessment.
• Providing psychosocial support to patient and family and educating them on appropriate care of patient at home as needed.
• Develop, plan, implement and evaluate nursing care plan for patients
• Carry out appropriate nursing care according to patient's needs
• Evaluate and monitor patient's progress in relation to care plan
• Provide timely interventions and treatment plans according to client's need

Requirements

• Candidate must possess at least a Diploma in Nursing
• Registered with Singapore Nursing Board
