Job Purpose
Singapore Red Cross operates the Red Cross Home for the Disabled (RCHD) and Red Cross Home Day Activity Centre (DAC), serving those with moderate/severe physical and intellectual disabilities.
Job Responsibilities
Social Work/Case Management
- Responsible for conducting/renewing means tests and financial assessment.
- Establish rapport and build professional relationships with clients and caregivers.
- Conduct initial screening and intake assessment to ascertain the level of complexity, key risks and protective factors of the case and prepare for necessary intervention that includes information and referral to help clients and caregivers.
- Prepare case history information, biopsychosocial assessment and social work intervention plans.
- Implement and follow up on care / case plans and interventions to determine the suitability of care / case plans in addressing client's needs.
- Plan, organize and lead admission/discharge planning and actively follow up with parents, RCHD's multidisciplinary team and external stakeholders with regards to the care plan.
- Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues of the residents, the goals to be accomplished and the appropriate social worker intervention.
- Assist residents and their families/significant others with attaining resources that fit their needs including the provision of financial assistance when required.
- Communicate and work closely with family members or caregivers for the clients care plan.
- Attend monthly meetings and recommend/highlight crucial cases if needed to the team.
- Develop individual care plans and carry out regular review of the cases.
- Conduct Client Assessment Form-Revised (CAF-R) evaluations for DAC clients.
- Prompt updates and documentation in the SW Communication Log Sheet for purposeful contacts.
- To keep appropriate case recordings and produce timely reports, as required, in accordance with departmental policies and procedures.
- Carry out any other duties as assigned by the supervisor.
Stakeholder Management
- Build and sustain collaborative working relationships with key ministries and social service partners such as MSF, SG Enable, SPED schools, service providers in the disability sector, and community partners.
- Participate in outreach to the relevant agencies in the disability sector.
- Understand and meet all government requirements for social service documentation.
Administration
- Covers DAC manager when he goes on long leaves.
- Handles the collection of DAC programme fees and transport fees.
- Prepares the various periodic reviews pertaining to statutory submission and audits.
- Handles the various social work reports: DAC Monthly Register, SW Monthly Report, SG Enable monthly data updates, etc.
Job Specifications
Qualification and Experience
- Recognized Degree/Postgraduate Diploma in Social Work.
- At least 3 years working experience in the social service sector and healthcare sector.
- Good knowledge in government means testing framework.
- Good case management skills.
Additional Skills and Knowledge
- Experience in preparing Audit Musters would be an advantage.
- Familiar in using Google Suite would be an added advantage.
