Job Description

The Community Intervention Team (COMIT) provides assessment, counselling, therapy, case management and psychoeducation support for clients with mental health issues and dementia, including their caregivers. COMIT works closely with the community outreach teams, General Practitioners, Polyclinics and other community partners to provide holistic care to clients.

Responsibilities
The Senior Social Worker conducts casework and group work as well as implement and evaluate community development activities and programmes to address service gaps in the community. As a Senior Social Worker, you will be involved in guiding the team to implement and evaluate casework and group work and organise strategic community development initiatives, programmes and research. Taking strategic direction from the senior leadership of VCS and reporting to the Division Head for DCHMH, you should be insightful, responsible and possess the willingness to supervise. You will also collaborate with other agencies and ministries in the course of your work and provide comprehensive support for team members to increase their effectiveness in their practice competencies.

Programme Implementation

. Provide clinical support for the implementation of COMIT programme according to Tiered Care model and ensure that service requirements are met.

. Network with key stakeholders to build collaborative partnerships and facilitate care and support for clients and caregivers.

. Provide direct care and support including assessment, intervention, and management of clients and/or caregivers.

. Provide clinical case supervision to Social Workers and support their professional development.

Uplift professional practice

. Assess current and anticipate emerging requirements to support strategic aims in consultation with Division Head of DCHMH and Division of Professional Practice & Development (DPPD).

. Drive self-care mechanisms within the division.

. Develop mental health supervision guidelines for the organisation.

. Co-lead professional development of staff specialising in mental health.

. Co-lead organisation research on current mental wellness/health programmes and specified issues.

. Involved in audits with adherance to organisation's and funding bodies audit framework.

. Propose enhancements to standards of care, organisational health policies, procedures and programmes in relation to community mental health.

. Provide inputs to inform community mental health policy changes and development at the national level.

. Co-lead improvements in models of care and service delivery within the organisation based on market research and emerging trends.

. Adapt crisis management approaches that enable the organisation to minimise risks during crisis situations.

Ancillary Duties

. Support the operations of Viriya Mental Wellness Hub located at Kent Road.

. Planning and executing events within the Division and organisation.

. Any other duties, as assigned.
