Senior Resident Care Coordinator (AN)

Assist in developing individualised care and support plans for residents according to their assessed needs and the Home's established procedures.

Support the implementation of care plans, monitor residents progress and report relevant changes or concerns to the appropriate personnel.

Organise and implement suitable programmes and activities to promote residents well-being, engagement and quality of life.

Conduct monthly resident interviews and maintain accurate records, including six-monthly review reports, for approximately 50 residents.
