Senior/ Physiotherapist
- To provide efficient physiotherapy sessions and develop innovative solutions to meet the client needs, in line with best practice. To work with a multidisciplinary team to ensure the coordination, development and delivery of a quality, client-centered therapy service.
- The physiotherapist will also be required to support other departments in providing clinical support where necessary, to ensure a seamless journey for our clients and their caregivers
Responsibilities:
- Clinical Duties
- Conduct accurate and holistic physiotherapy specific assessment for clients during admission, periodic care plan reviews and discharge
- Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making
- Formulate person centered and SMART goals in collaboration with clients and relevant stakeholders
- Design plans and conduct physiotherapy interventions for clients in a person centered manner, that is also evidence based
- Document assessment results, therapy plans, interventions, goals and outcomes measures according to guidelines
- Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholders
- Ensure proper use of equipment including technological-assisted rehabilitation equipment
- Formulate discharge and continuity of care plans based on clients performance in consultation with relevant stakeholders
- Facilitate community integration through collaboration with other community partners as required
- Empower clients and caregivers with methods to enhance health, mobility and functions through self-management strategies.
Requirements:
· Diploma or Degree in Physiotherapy and must be AHPC registered
· Minimum 2 years of neuro-rehabilitation experience
· Good team player and able to work independently
