Job Description

ROLES AND RESPONSIBILITIES

  • Provide advanced bio-psychosocial assessments and interventions for patients / residents / clients / caregivers, including the management of complex and specialised cases in accordance with professional standards
  • Deliver end-to-end counselling and case management, including comprehensive assessment, care planning, intervention, and evaluation to ensure appropriate levels of care
  • Lead the development, facilitation, and evaluation of group work programmes to build social support systems and strengthen community networks
  • Identify service gaps and emerging needs advocate for and lead the design and implementation of targeted programmes to improve service delivery for patients / residents / clients / caregivers and relevant stakeholders
  • Plan, manage, and implement specialised programmes and services involving patients, families, multidisciplinary teams, and community partners
  • Provide team leadership and clinical guidance to ensure delivery of high-quality psychosocial care, in alignment with the Code of Ethics, Standards of Practice, and organisational governance
  • Supervise and mentor Medical Social Workers, interns, and support staff to support professional development and uphold standards of practice
  • Lead and contribute to departmental and organisational initiatives, including process improvement and quality enhancement efforts to better meet client and caregiver needs
  • Develop and deliver skills-based training and education for MSWs, healthcare professionals, and community partners in specialised areas of practice
  • Remain vigilant in identifying potential risks in practice and care delivery, and support organisational risk management efforts by taking appropriate actions to ensure safe, effective services in alignment with Ren Ci Hospital's policies

JOB REQUIREMENTS

  • Degree in Social Work (or major in Social Work) from a recognised institution Bachelor's, Postgraduate, or Master's qualification
  • Accredited as a Practicing Social Worker with the Singapore Association of Social Workers (SASW)
  • 5-8 years of relevant experience in social work practice, preferably in healthcare or community settings
  • Strong experience in case management, including psychosocial assessment, care planning, and complex case intervention
  • Experience working in multi-disciplinary teams, with exposure to programme development or group work facilitation
  • Strong social casework and intervention skills, particularly in managing complex and specialised cases
  • Good counselling and emotional support skills
  • Effective interpersonal and communication skills, with ability to engage diverse stakeholders
  • Demonstrates self-awareness and ability to use self effectively in facilitating change
  • Strong mentoring and supervision skills to guide junior staff and interns
  • Good organisational and time management skills, with ability to manage multiple priorities
  • Proficient in Microsoft Office (Word, Excel) for documentation and reporting
  • Able to identify and manage practice risks effectively
  • Proactive, adaptable, and committed to continuous learning and service excellence
