Job Scope:
- Conduct socio-economic assessments to support patients and families with financial and practical needs
- Provide ongoing counselling (individual/family) through face-to-face sessions and home visits
- Collaborate with community partners (educational, religious, social services) to support end-of-life care goals
- Support and prepare patients and families for end-of-life, including care planning and emotional readiness
- Participate in interdisciplinary care conferences with healthcare teams for holistic patient management
- Advise on financial assistance and discharge planning
- Perform fieldwork across hospitals and community agencies to address medico-social issues
- Prepare reports and present complex cases for discussion and review
- Manage complex casework and document all interventions within required timelines
- Provide clinical supervision and professional input when needed
Requirements:
- Minimum Degree in Social Work and / or Master in Counselling preferably
- Registered with the Singapore Association of Social Workers for Social Workers or Singapore Association for Counselling (SAC).
