- Provide and facilitate socio-economic evaluations to help patients and their families address practical concerns, such as financial matters, which are often an issue at the end of life
- On-going individual and family counseling through face-to-face sessions
- Work with the broader community which includes educational, religious, and social service organizations so that patients and their families can achieve the optimal care plan
Requirements
- Degree/ Graduate Diploma in Social Work
- Registered with the Singapore Association of Social Workers
- Passion to serve and provide good patient care
- Good interpersonal skills and able to work independently
- Flexible, adaptable, and able to work effectively in varied settings
