Key Responsibilities- Support the strategic planning and development of the integrated community care system by collaborating with operational and clinical teams to
- Develop community care models targeting local needs and supported by international best practices
- Design and implement innovative strategies to strengthen hospital-community integration.
- Transform strategic plans into practical, actionable work processes that enhance service delivery.
- Support project performance management and analytics
- Develop and track key performance indicators for community care initiatives
- Provide strategic insights and recommendations based on programme outcomes
- Preparation of updates and present analytical reports to senior management to guide decision-making
- Lead project planning and management
- Lead and manage projects from conception to implementation including project plans, budget, resource planning, timelines and deliverables.
- Drive cross-functional initiatives involving multiple internal and external stakeholders
- Support in stakeholder engagement and build partnerships
- Build partnerships and facilitate collaboration with key stakeholders e.g. NUHS institutions, government agencies, community care providers.
- Lead strategic discussions and working groups, provide secretariat and administrative support to facilitate alignment of strategies and support execution of social health care initiatives.

Key Requirements
- Bachelor degree in any discipline
- Strong analytical abilities
- Project management skills
- Strong written and verbal communication abilities
- Adept in people skills to foster buy-in and collaboration
- At least 2 years of working experience
- Knowledge and experience in project management will be advantageous
- Experience in the Singapore healthcare system and/or knowledge of integrated care models will be advantageous
