As part of the Advance Care Planning (ACP) team, you will support and drive the implementation of the ACP programme in Singapore.
- Support and co-lead the engagement and recruitment of new partners/Service Providers (SPs) in the community care sector to increase touchpoints for Advance Care Planning
- Undertake role of Programme Manager (which includes fund administration) for assigned community nodes service providers and to work collaboratively with the SPs to achieve
desired programme outcomes - Support capability development and training of comm nodes service providers
- Co-develop and implement public education and outreach initiatives to raise ACP awareness
- Support projects and initiatives related to ACP implementation in the community nodes
- Provide secretariat support for ACP related workgroups and committees
Job Requirements
- Degree in any field
- Prior experience in acute care or community settings and/or a project management role would be advantageous.
- Possesses strong interpersonal and communication skills (written and spoken), knowledge in data analytics and is meticulous
