Job Description

Brief Summary

Join a dedicated team as a community-based dietitian, focusing on enhancing the health of individuals and families through targeted nutrition therapy and innovative program development.

Responsibilities

Provide medical nutrition therapy through home visits, telehealth, and outreach initiatives. Collaborate with multidisciplinary teams to develop and implement nutrition programs. Evaluate the effectiveness of nutrition education resources to improve community health outcomes. Engage with clients to conduct assessments and design personalized care plans.

Requirements

Bachelor's Degree in Nutrition and Dietetics. A minimum of 2-3 years of experience in community or public health nutrition and clinical dietetics. Strong competencies in client assessment, care planning, and telehealth service delivery. Demonstrated ability to collaborate effectively within interdisciplinary teams. Excellent communication and training skills, along with stakeholder engagement capabilities.
