JOB PURPOSE
You will be part of our community outreach team, also known as the Community Resource, Engagement and Support Team (CREST), focusing on raising public awareness of mental health conditions and dementia, early identification of at-risk individuals, providing emotional support to individuals and their caregivers, and linking individuals to relevant health and social care services where necessary. You will be placed within the Community Mental Health Department, under the Mental Health & Counselling Services Group.
KEY RESPONSIBILITIES
Clinical Leadership & Supervision
- Provide clinical leadership for the implementation of the CREST programme according to the Tiered Care Model, and ensure service requirements are met
- Oversee clinical case supervision and staff development of social workers
- Monitor supervisees compliance with professional processes (such as the use of tools and templates, and adherence to practice policies)
- Conduct coaching and performance management reviews with staff
- Induct new hires in the centre through individual supervision during their probation period
Case Management & Direct Practice
- Provide case management services across assessment, case management, systems work, case conferences, and closure, and maintain proper and up-to-date case documentation
- Attend to enquiries and provide information and referral services
Programme & Operations Management
- Establish and operationalise programme-level SOPs and clinical workflows
- Plan, coordinate, execute, and evaluate ongoing programmes and services, including outreach, on-site, and online public education
- Oversee the tracking of programme data and prepare submissions of reports to the programme funder
- Support the Service Head and Principal Social Worker in the implementation of the centre's operational and practice protocols, processes, and policies
Stakeholder Engagement & Partnerships
- Engage key community stakeholders to coordinate and implement mental health programmes
- Network with community stakeholders to build collaborative partnerships and facilitate care and support for clients and caregivers
Other Duties
- Opportunities to be involved in organisational-level projects
- Undertake any other duties as assigned by the Service Head
QUALIFICATIONS
- Bachelor of Social Work or Postgraduate Diploma in Social Work from an accredited institution
- Registration with the Singapore Association of Social Workers (SASW) is preferred
REQUIREMENTS
- 6 to 8 years of relevant experience in healthcare or community care settings
- 3 to 5 years of direct clinical practice working with individuals with mental health and/or dementia-related conditions
- At least 3 years of supervisory experience
- Demonstrated competence in engaging, assessing, intervening, and evaluating social work practice
- Ability to understand client groups and manage risk and protective factors
- Good knowledge of community resources, referral pathways, and service systems, with the ability to advocate for clients
- Ability to lead and manage a team of social workers to achieve programme outcomes
- Strong organisational skills with a structured and systematic approach to work
- Strong proficiency in written and spoken English, required for effective facilitation, presentation, and professional communication
- Strong interpersonal and communication skills to engage effectively with individuals and systems at multiple levels
- Collaborative team player with the ability to work effectively in a dynamic and fast-paced environment
- Demonstrated commitment to mental health and community-based work
- Proficiency in an additional language is an advantage, as it may support engagement with service users
