Senior Assistant Manager / Manager, Partnership & Community Development

Overview

As a member of Planning and Development Department , you will play a key role in shaping this ecosystem-bringing internal and community stakeholders together, coordinating initiatives, and integrating health and social support systems to enhance the delivery of community care and improve health of the population. Job Responsibilities

• Support in planning and operationalizing of Health Precincts concept to achieve health-social integration with internal and community stakeholders
• Support in developing and operationalizing plan to empower community to take care of their health.
• Develop and implement community health plan for the precincts under your care, with emphasis on preventive health and social-health integration
• Track and review progress on implementation of community health plan for different focus areas
• Engage & build partnership with internal and community stakeholders e.g. grassroots organizations, social service agencies and healthcare providers to keep patients and residents healthy and integrated care across care settings
• Any other duties, projects or assignments by Reporting Officer and/or HOD

Requirements

• Bachelor degree in any discipline.
• Minimum 5 years of working experience.
• Strong planning, project management and organizational skills.
• Adept in stakeholder engagement to foster buy-in and collaboration.
• Exceptional written and verbal communication skills
• Resourcefulness in problem solving and resilient in overcoming challenges.
