Job Description
Assist the therapists in assessing clients status for suitability to start therapy sessions or continue with therapy sessions
Conduct routine assessment as per therapist instructions.
Document results of case history taking, assessment results, therapy plans, interventions, goals and outcome measures to be reverified by therapist in charge
Prepare clients, materials and equipment for assessments or therapy
Communicate information of therapy sessions to the therapists
Conduct dementia related assessments as instructed by therapist
Perform routine clinical interventions
Train client in assistive devices and equipment according to therapist instruction
Provide clients and caregivers with education and training according to therapist instruction
Inform therapist if client's condition changes.
Conduct basic or routine group therapy
Assist in conducting dementia related interventions
Communicate information of the therapy session to therapist
Document therapy session according to template set for approval by therapist
Be a resource person on the operation of rehab equipment in the centre
Maintain cleanliness and safety of therapy areas
Adhere to infection control procedures
Manage feedback from staff, clients, and NOK - To escalate to Head, Rehab accordingly.
Work with Head, Rehab to provide service recovery for client complaints
Manage adverse events in unit in line with existing SOPs
Identify gaps in clinical practice
Participate in value based care projects
Participate/ Contribute to professional continuing education
Participate in research projects
Provide statistics and reports as required
Undertake department administrative duties such as inventory, ordering stationary and routine supplies within budget
Assist in the planning and execution of department events such as rehab day, team bonding, etc
Requirements
WSQ Therapy Support or equivalent
Less than 2 years relevant experience
