Job Description

As a Programme Coordinator for NUHS@Home, you will play a vital role in the coordination and management of our patients care.We're looking for people with new ideas and a can-do attitude. Job Responsibilities

  • Involve in the planning, assessment, and monitoring of our program to ensure that we are providing the best possible care to our patients

  • Coordinate with external vendors to arrange for transport and courier deliveries

  • Coordinate with NUH & cross hospital BMU to manage bed requests

  • Coordinate with multi departments across NUHS to schedule appointments & other services as requested by the clinicians

  • Perform patient admissions, discharges and transfers on Electronic Medical Record (EMR) system

  • Conduct financial counselling to patient/NOK on the estimated NUHS@Home bills, available subsidies, various payment schemes and mode of payment.

  • Assist patient to complete various modes of payment forms. Ensure proper documentation and follow up for all inflight case until patient discharge.

  • Export reports from electronic medical records system and import data into relevant channels timely to facilitate information retrieval for the clinical team.

Requirements

  • Diploma in any discipline

  • Able to converse in English & mother tongue language (Chinese/Malay/Tamil)

  • Computer literate and be proficient in Microsoft applications

  • Be willing to work outside of working hours (E.g. weekends and/or public holidays)

  • Able to cope with and work in a fast paced environment

  • Must be a good team player with positive work attitude

  • Prior knowledge/working experience in a hospital setting is an added advantage.
