Post Transplant Coordinator (Kidney Transplant)

We are seeking a dedicated and compassionate Post‑Transplant Coordinator to join our Kidney Transplant Programme. This role plays a vital part in supporting kidney transplant recipients throughout their long‑term recovery and ongoing care.

Key Responsibilities

• Conduct patient interviews and perform preliminary health assessments
• Coordinate investigations, clinic appointments, and follow‑up care
• Monitor medication adherence and provide patient counselling
• Educate patients and families on self‑care and post‑transplant health
• Maintain accurate patient records and support data reporting
• Participate in quality improvement initiatives and research activities

Requirements

• Degree in Nursing, Psychology, or a Health‑related field
• Experience in healthcare or other people‑centric roles
• Strong communication, teamwork, and patient‑education skills
• Willingness to perform on-call duties on a rotational basis
