The Role
The Physiotherapist at the Tsao Foundation's day rehabilitation centre plays a crucial role in the healthcare team, specifically focusing on the physical health of elderly clients. Your main purpose is to conduct thorough physical assessments to identify and address the challenges and limitations that come with ageing. You will then develop and implement individualised treatment plans and functional mobility training to restore physical function, improve balance, and prevent falls. By doing this, you directly enhance the overall well-being and functional independence of our elderly clients, helping them to live better and more independently in their own homes and community.
Key Responsibilities
- Evaluate clinical history and conduct client assessment for the participant, including physical, impairment-based assessment and relevant outcome measures.
- Develop client centred and SMART goals, treatment plans and implement these plans for participants and caregivers.
- Provide appropriate physiotherapy interventions and client education based on assessment, including; Functional mobility training/ Gait aid prescription, Pain management, Balance Training, Strengthening/ Endurance training and more.
- According to participant's needs: Plan, conduct or supervise, and evaluate individual and group physical activity and exercise programmes, OR plan individual and group physical activity and exercise programmes for Programme Assistants [PA] and/or volunteers to conduct.
- Conduct education and training for caregivers
- Conduct home therapy when required
- Manage emergent and other situations such as falls, medical incidents, client complaints, as they arise.
- Assist in implementing new clinical services/ therapy including the use of rehabilitation technology.
- Participate in continuing education and research including delivering in-service training for staff and volunteers and participating in quality improvement initiatives
- Maintain timely and accurate documentation of participant's progress and outcomes of care.
- Meet key performance indicators (KPIs) and provide accurate reports as required by funders/ auditors/ management in a timely manner.
- Other duties as directed by the HOP and/or Lead Therapist.
Skills and Knowledge
- Registered with the Allied Health Professionals Council. (Full Registration)
- Minimum 2years experience as a physiotherapist
